Sažetak
The activity of alkaline phosphatase (AP) was measured in 59 peroral small intestine biopsies from 53 patients with coeliac disease (CD) and controls. According to clinical picture and histological findings of small bowel biopsies, patients were divided into six groups. Control group with normal histological and AP findings, patients with coeliac disease - 1st biopsy on normal diet, patients with coeliac disease - 1st biopsy after 3-4 weeks of a gluten free diet (GFD), patients with coeliac disease - 2nd biopsy after 1-3 years of a GFD, patients with coeliac disease - 3rd biopsy 3-6 months after challenge with gluten, patients with acute intestinal disorders on normal diet. AP activity depends on the degree of critical state of pathological process and decreases if the process is more acute. Decreased AP activity, independent of the cause, points to its nonspecific character. The results presented in this study showed that, by measuring AP activity and by histological examination of intestinal mucosa 2-4 weeks after the begining of GFD, and by reintroduction of gluten 2-3 years later, the second biopsy can be avoided. In that way the reliability of CD diagnosis would not be affected, but the inconvenience and dangers of biopsy will be reduced.
