BACKGROUND: Adolescence is the age of sexual development and seeking partners, and perception of facial attractiveness plays an important role in the process. Therefore orthodontist must be aware of facial profile preferences because his therapy can alter attractiveness of the facial profile lower third. AIM: The aim of this study is to establish which facial profiles mostly discriminate perception of male and female adolescents. SUBJECTS AND METHODS: Random sample of 1089 adolescent was used (450 male and 639 female) aged 15-19 years (16.3±1.4) from 12 secondary schools in Zagreb, Croatia. Eight profile distortions for each gender, morphed by a digital imaging technique, were assessed for attractiveness by each adolescent and scored from 1 to 8. Multivariate discriminant analysis was used for statistical analysis. RESULTS: In general male and female adolescents have similar facial profile preferences. Straight profile is considered the most attractive for both genders. Both bimaxillary alveolar protrusive and retrusive profile are equally attractive among male profiles, but in female profiles the ticker than flat lips are significantly more attractive. Extreme convex profile is least attractive, but mild convex shape is more preferable than concave. According to discriminant analysis males and females are mostly discriminated in attractiveness scores of both bimaxillary alveolar retrusion and protrusion, mandibular retrognathism in female profiles, and combination of maxillary prognathism and mandibular retrognathism in male profiles. In comparison to males, females are less tolerant to retrusive lips in females and convex profiles in both males and females. DISCUSSION: There is a general preference among adolescents for an orthognathic profile but they are more tolerant of prominent chin in men than in women. Females tend to pay more attention to facial appearance and details on profiles of their own gender, particularly in lip prominence, than man do. Therefore, contrary to popular belief, fuller lips in females are more appreciated by females than males. Orthodontic therapy that results in flatness of lips and pronounced mandibular deficiency in female profiles will probably more affect female self-esteem and lead to perceived loss of beauty in women’s perspective than in men’s. CONCLUSIONS: Due to facial profile preferences in borderline cases it would be wise to treat female patient with non-extraction mode, while in male patients we can more easily decide to proceed with extraction treatment.
