Background: According to norm HRN EN ISO 15189 the laboratory staff was trained to identify and record preanalytical nonconformities encountered within received specimen and to record on the test report if the quality of sample was unsuitable for examination. These nonconformities were declared as test request form or samples nonconformity and were reported monthly. Methods: The frequencies of nonconformities in Emergency laboratory of University department of Chemistry "Sestre Milosrdnice" University Hospital were assessed and compared during the two subsequent periods from April 2007 to December 2008. A total of 210, 419 samples were received over the study period. During the first period records on nonconforming request and samples were recorded using paper forms. During the second period, the nonconformities were recorded electronically through laboratory information system (LIS). To assess the efficiency of our system we compared the number of LIS recorded nonconformities with the number of test reports with comments pointing to some preanalytical nonconformity for randomly selected one week period (2538 samples). Results: During the first period of time there were recorded 3.2% preanalytical nonconformities altogether. During the second period we observed 30.1% and 0.7% nonconforming request and samples respectively. The difference between periods was statistically significant (p<0.001). During the randomly selected one week period 0.7% nonconforming samples were recorded and 3.7% test reports with comments pointing to some preanalytical nonconformity (p<0.001). Conclusions: Although information technologies significantly improved the quality system documentation, according to result of randomly selected data it is obvious that further improvement is obligated.
