Sažetak
Intramyocardial course of coronary arteries – myocardial bridging is a congenital anomaly almost exclusively confined to the mid portion of the left descending coronary artery (LAD). It is found in 1.5-16% of patients undergoing coronary angiography whereas some autopsy series have reported the incidence to be as high as 15-85%. Myocardial bridging is clinically manifested as typical or atypical angina, myocardial ischemia, myocardial infarction, left ventricular dysfunction, myocardial stunning, AV conduction disturbance, exercise-induced ventricular tachycardia and sudden death. However in a great majority of patients it represents a relatively benign disease. Coronary angiography, intravascular ultrasound and Doppler flow measurement, fractional flow reserve (FAR) determination, myocardial perfusion scintigraphy, SPECT, MSCT angiography and MR have significantly improved and refined the diagnosis of myocardial bridging. However, the accuracy of the multimodal assessment of clinical significance of myocardial bridging same as the optimal treatment of choice in an individual patient remains controversial. In this study we present the 5 year follow – up experience with PCI using DES in eleven patients with symptomatic myocardial bridging.
