Sažetak
Methadone pharmacotherapy has been introduced in Croatia in mid nineties and the expenses have been covered by the Health Insurance Institute for years. The document „ The National guidance for the methadone pharmacotherapy of opiate addicts“ has been adopted by the Croatian Government in 2006. and published at the web site of the Ministry of Health and Social Welfare. Buprenorphine has been gradually introduced in the treatment programme since 2003, and since 2006 the expenses are covered by health insurance. The similar document on buprenorphine pharmacotherapy was adopted in 2006 as well. In the process of guideline creation the representatives from the Ministry of Health and Social Welfare, Croatian Office for Combating the Narcotic drug Abuse, Referent Center for Drug Abuse, Croatian National Institute of Public Health and Society for Family Medicine Croatian Medical Association were involved and worked together. The accurate relevant documents by WHO, EMCDDA, NIDAS and EK were used as the framework. The specific Croatian situation with out-patient centers being part of the health system and family physicians being responsible for daily dose taking supervision, have been described precisely and the role of the each segment defined. The main goals were to include the heroin addicts early in the treatment system, to cover with the treatment as big proportion of heroin addicts as possible, and to maintain the treatment not allowing them to leave before ready. In the guidelines the protocol of inclusion criteria, treatment, dosage regiment and daily supplies were described and recommended. The firs draft was discussed at the professional meetings and at the Governmental Committee for Drug Addiction Prevention. After approval was obtained, the Ministry of Health started the necessary legislative procedure and the Guidelines were adopted by the Government. The adoption of the guidelines does not disturb other treatment methods as psychocial support, psychotherapy, counseling or drug free approach.
