Sažetak
Cutaneous angiosarcoma is a rare delayed complication in patients who undergo breast conserving surgery combined with radiotherapy. In this case report we present a 59 year old women, who in year 2001 underwent a quadrantectomy of the left breast. Resection margins were clear and afterwards the patient was treated with radiation therapy and 6 cycles of chemotherapy (FEC). Seven years after initial presentation she complained of discomfort in the left breast. Clinical examination, mammography, ultrasound and cytology revealed no evidence of recurrence. Three months later the patient complained of increasing swelling and pain. Two skin biopsies were performed and histology showed atypical vascular proliferation. Another three months later new skin lesions appeared, and the patient was sent to our dermatology clinic. On examination the left breast was oedematous with bluish papules, few haemorrhagic blisters, and indurations. Three large incision skin biopsies were performed. Histologically, skin biopsy showed irregular, small vascular spaces dissecting superficial and deep portions of the dermis. Endothelial cells focally demonstrated significant nuclear atypia and even epithelioid morphology. In the subcutaneous fat tissue, a part of bigger vascular space was lined with several layers of large, atypical endothelial cells. Immunohistochemically tumor cells stained diffusely positive for CD31 and CD34. In conclusion, due to increased number of breast conserving therapy combined with irradiation, in some cases cutaneous angiosarcoma appears. The aggressive nature of this disease demands careful follow-up, early diagnosis and radical treatment.
