Sažetak
Normal physiological aging processes are related to cognitive decline. Moreover, older adults are at higher risk of cognitive impairment than the rest of population and cognitive functioning is especially likely to decline during illness or injury. Oxidative stress, which is implicated in the pathogenesis of over 100 human diseases, is shown to be associated with cognitive impairment. Trace element selenium is an integral part of more than 30 proteins and have an important role in protection against oxidative stress: glutathione peroxidase (GPx) is an antioxidant selenoenzyme that reduce potentially damaging reactive oxygen species (especially important as the brain is deficient in catalase) and it is required for appropriate thyroid hormone synthesis, activation and metabolism (even subtle changes in thyroid function may affect cognition). This study considers possible associations of biomarkers of selenium status with a reduced risk of cognitive decline in elderly subjects from coastal (Vis Adriatic island ; VA) and continental areas (region of Virovitica ; RV) of Croatia. The regions are characterized by specific (Mediterranean and continental) life style and dietary habits. The sample comprised representative sample of predominately rural population that was a part of the 4th follow-up (in 2006) of a larger longitudinal study on the prevalence of chronic diseases in Croatia (years held: 1969, 1972, 1982, and 2006/07): 37 elderly subjects (20 women and 17 men, aged 71-87) from Vis Adriatic island (VA), and 63 subjects (38 women and 25 men, aged 71-88) from continental region of Virovitica town and surroundings (VR). The study was performed in accordance with the ethical principles of the Helsinki Declaration and approved by the Institute’ s Ethical Committee. Mini Mental State Examination (MMSE) was used to assess cognitive function. GPx activity in whole blood was measured by the EC-FLAIR common assay. Serum selenium was analysed using electrothermal atomic absorption spectroscopy (Perkin Elmer Aanalyst 600). Accuracy of the measurements was checked using certified reference material (BCR 637-639) and through regular participation in the UK TEQAS programme. The results showed a significantly higher SSe (p<0.0001) and GPx (p<0.0001) levels as well as the higher (p<0.017) score of MMSE test among the subjects from coastal area (VA) in comparison to RV subjects. Increase in MMSE score was significantly associated with an increase in SSe (Spearman r=0.30, p<0.003). GPx significantly correlated with an increase in SSe in all subjects (r=0.55 ; p<0.0001), as well as only in VA (r=0.39 ; p<0.03) and RV (r=0.26 ; p<0.05) subgroups. Higher SSe level in subjects from VA coastal area is probably due to higher consumption of fish in comparison to subjects from continental RV area (SSe significantly correlated with increase in fish (r=0.34 ; p<0.05) and decrease in meat (r=-0.37 ; p<0.03) consumption reported by subjects). There were no significant differences between the groups in age, BMI, alcohol intake or smoking habits. Significant positive association that was found between SSe and cognitive scores indicate and confirm a role that selenium may play in the protection from cognitive decline.
