Brain tumours can cause symptomatic epilepsy in 30-40% of cases. The aim of this study was to analyse frequency of symptomatic epilepsy as the first symptom in patients with primary brain tumours. We have gathered 849 patients with primary brain tumour. Tumours were grouped according to classification of the WHO. Patients were divided by gender, age, clinical features and neuro-radiologic findings, histological type of tumour and its location. From total number of patients with primary brain tumour, 182(21.4%) of them have had an epileptic seizure as a first symptom of tumour, 102(56%) men, and 80(44%) women, respectively. The highest frequency of seizures appeared in sixth decade of life. Types of tumours were different, as a histological result, astrocytomas type I and II (38%) were the most frequent. Complex partial seizures appeared in tumours located in the temporal lobe and generalized seizures appeared in those patients who had tumour located in frontal and parietal lobes. In 7 (3.8%) patients with epileptic seizures initial MRI was normal. The results of this study confirm that epileptic seizures depend of localization of the tumour, and appear more often in particular type of tumours. Epileptic seizures in the elderly especially partial or generalized with focal origin, need to be followed up, at lease for two years, disregarding normal initial MRI finding.
