Recently it has been suggested that a combination of functional fibrinogen and D-dimer measurement (D/Fratio) above certain cut off is highly specific for the presence of thromboembolic events among patients with clinical symptoms of the disease. We investigated, retrospectively, D/Fratio preformance in patients with hip injuries referred to our hospital during one year. D-dimers and fibrinogen were measured in plasma samples by immunofiltration (Nycocard ReaderII, AxisShield, Norway) and Clauss method (Dade Behring ; Sysmex CA1500 analyzer), respectively. Data were reported as proportions and medians with range, and as appropriate were compared by the Mann– Whitney test. On admission, obtained D-dimer values, fibrinogen and D/Fratio in 40 consecutive patients were 2.6(0.6-5.5) mg/L, 3.5(2.9-4.6)g/L and 0.8(0.2-1.7), respectively. According to comprehensive physician report and doppler ultrasonography, two groups were considered: GI, group of patients(26/40) without and GII, group with clinical symptoms(14/40). Although without statistically significance(p 0.05), initially observed DD and D/Fratio values were higher in GII than in GI patients (4.4vs1.8 mg/L ; 1.0vs0.6, respectively), while fibrinogen levels were the same (3.5vs3.7, p=0.799). After surgery, in both groups, DD and D/F ratio values were declined (<1.5mg/L ; <0.3, respectively) and they didn´t changed. The lowest initial D-dimer and D/Fratio values (1.3mg/L ; 0.3, respectively) were observed in six GII patients with confirmed DVT during observation and remain the same until end of observation. Collected data do not allow us to make definitive conclusions because small nuber of patients, but it seems that D/Fratio among the high probability patients could not provide a further informations. To ascertain this, further prospective observations are necessary.
