Sažetak
Purpose. To assess the value of ultrasound (US) in differentiation benign from malignant parotid tumors and benign non-tumorous lesions from benign tumors. Methods. During a 3-year period, sonographically guided fine-needle aspiration biopsy (FNA) was performed on 89 suspected parotid lesions with a size ~ 7 mm. We obtained 80 (90%) specimens adequate for cytologic analysis and 9 (10%) non-diagnostic specimens. We recorded the following 7 sonographic parameters: size, number, echogenicity, echotexture, margin, distal acoustic enhancement, and regional lymph node enlargement. Results. FNA diagnoses included 32 (40%) non-tumorous lesions, 30 (38%) benign tumors and 18 (22%) malignant tumors. The mean size of the malignant tumor was 25± ; 17 mm versus 27± ; 17 mm for benign tumors versus 21± ; 12 mm for non-tumorous lesions (p > 0.05). Among 54 solitary lesions, 9 were malignant tumors, 24 were benign tumors, and 21 were non-tumorous lesions (p > 0.05). The majority of the all parotid lesions were hypoechoic. The sonographic features that suggest malignancy were: heterogeneous echotexture, indistinct margin, and regional lymph node enlargement. The sonographic feature that suggests benignancy was distal acoustic enhancement. Conclusion. Benign and malignant parotid lesions have a similar sonographic appearance. Heterogeneous echotexture, indistinct margin, and regional lymph node enlargement suggest malignancy. Distal acoustic enhancement suggests benignancy. Sonegraphically guided FNA should be performed on parotid lesions when a tumor is suspected.
