Sažetak
A 61-year-old white man was seen several times during the last five years because of repeated respiratory tract infections. In February 1992 he developed pernicious anaemia without antiparietal antibodies (E 1.22 ; Hb 54 glI ; MCV 123.5 ; T 49 L 3.6) and malabsorption syndrome one year later. Small intesti¬ ; ne biopsy specimens did not give diagnosis (blunting of villi was present focally). The patient~ eczematoid dermatitis per¬ ; sisted with efflorescences often becoming pustulous. Laboratory results were in agreement with the state of immu¬ ; nodeficiency: proteins 57.0 g/l, globulins: 7.8-13.6-11.7¬ ; 10.0% ; IgG 5.0, IgA 0.5, IgM 0.6 glI: immunophenotypes of lymphocytes (%): CD2 98.9, CD4 31.4, CD8 55.0, CD19 0.2, T4/T8 = 0.57. Coprocultures and parasites in stool were negati¬ ; ve. Echography visualized enlarged spleen (14 cm in diameter) with unconspicuous cytology. Reviewing chest X-rays from 1989 to 1993, a mass of 5 cm in diameter could be discerned in the upper anterior mediastinum. Repeated cytology punction gave no conclusive answer about the nature of this lesion, obviously benign by its course. Computed chest tomography defined it as a solid, partly necrotic and well encapsulated for¬ ; mation. A diagnosis of thymoma accompanied by hypogamma¬ ; globulinaemia was established.
