Sažetak
Cardiovascular effects of involuntary breathing movements that occur during the struggle phase of a prolonged apnea are unknown. In this study we examined whether the reduction in stroke volume during a maximal dry breath-hold is related to declining venous return and whether involuntary breathing movements can attenuate this effect. We measured central and peripheral hemodynamic parameters as well as splenic volume before, during and after two maximal dry breath-hold attempts at rest in seven elite apnea divers. During the early easy going phase, in which heart rate remained constant, cardiac output was reduced by approximately 40% due to reduction in venous return and stroke volume. The large reduction in stroke volume and cardiac output was offseted in the struggle phase by the start of involuntary breathing movements, with normalization of stroke volume and cardiac output at the end of this phase due to improved venous return. Arterial pressure was progressively increased initially due to peripheral vasoconstriction, and later due to involuntary breathing movements. Involuntary breathing movements caused mechanical compression of spleen for about additional 70 ml, thereby increasing the peripheral blood volume. We believe that involuntary breathing movements have consequences on cardiovascular function during the later phase of apnea.
