Sažetak
The correlation between the severity of periventricular leukomalacia (PVL) and epilepsy has not been fully clarified. We studied the relation between various grades of mental retardation severity in PVL and West's syndrome (WS). This study included 37 patients born at 28-32 weeks of gestation. We performed brain ultrasound and MRI to verify PVL, and these patients were then classified according to De Vries. WS was classified according to the International League Against Epilepsy 1989. All our patients were premature infants, born at the mean of 33.3 week of gestation. WS developed during the first 2 years of life, with the peak age at onset of 6-8 months. The most common interictal abnormality was hypsarrythmia (62%). All patients except for two showed developmental delay and a varying grade of mental retardation. Neuroimaging findings showed PVL type I in two, PVL II or PVL type III in 31, and PVL with intraventricular hemorrhage in four patients. The most characteristic neuroimaging findings in premature infants with PVL and WS were: major reduction in cerebral cortical gray matter volume ; reduction in the volume of brain myelin ; delayed myelination ; and atrophy of the midbrain and pons. These findings may imply an anatomical correlate for the onset of WS in association with PVL. Also, they may account for the intellectual and cognitive deficit in premature infants with PVL.
