Atopic dermatitis (AD) is a chronic inflammatory skin disease, occurring more commonly in children. In recent years it has become increasingly clear that eosinophils play an active role in the pathogenesis of atopic diseases. Eosinophil cationic protein (ECP) is a cytotoxic agent secreted by activated eosinophils during allergic and inflammatory processes. Elevated levels of ECP have been detected in serum, sputum or nasal secretion during allergic reactions, therefore, serum concentrations of this protein could be a helpful in vitro parameter in monitoring disease activity in patients with AD. For the evaluation of patients with AD we performed standard diagnostic procedures and measured serum ECP levels (UniCAP FEIA, Pharmacia). The secverity of AD was objectively assessed by the SCORAD Index, developed by the European Task Force on Atopic Dermatitis in the year 1993. Fourteen children with moderate to severe forms of AD, younger than 3 years of age, were evaluated. Median serum levels of ECP were 17.1 ug/l (ranging from 3.01 to 57.9 ug/l), which were much higher, compared to results of serum ECP in normal non-atopic children. In conclusion: measurement of serum ECP could be a good marker for active inflammation with eosinophil involvement in AD patients.
