Cytology of peritoneal washings in borderline ovarian tumors Sentija K.1, Krivak Bolanca I.1, Gasparov S.2 1Laboratory for Clinical Genetics and Cytology, Dpt Gyn and Ob, 2Dept for Cytology and Patology, Clinical Hospital Merkur, Zagreb, Croatia Background: Peritoneal washings play an important role in the diagnosis and prognosis of ovarian tumors. In our clinic, cytological analysis of intraoperative peritoneal washing and cyst fluid specimen is routinely performed. Objectives: We present the results of cytological analysis of peritoneal washings and cyst fluid for fourteen patients with cystic ovarian tumors, operated in our clinic in the last 5 years. Methods: All patients but one are in generative age, with ovarian cysts ranging from 0.6 to 15 cm in diameter, and with tumor marker Ca125 ranging from 9.2 to 40.3. Intraoperativelly peritoneal washings was taken for fourteen, and cyst fluid specimen for nine patients. All cysts had an intact smooth surface, except for two cysts with papillary formations, and all were histologically confirmed as atypical proliferating tumors with unknown malignant potency or borderline tumors. In five washings, to distinguish atypical tumor cells from reactive benign mesothelial cells, immunocytochemical reaction on epithelial antigen (EA) was performed. Results: During the abdominal surgery, cyst fluid was cytologicaly examined for nine patients: in one case we found normal cylindrical epithelia ; six cysts showed significant atypical proliferation of cylindrical cells, partly with papillary appearance, with polymorphism, overlapping and hypercromasia of nuclei ; in two cases we found papillary clusters of malignant cylindrical cells. Intraoperative examination of peritoneal washing was performed for fourteen patients. Six patients showed no abnormalities, with the presence of only reactive mesothelial cells. In three cases we found normal cylindrical epithelia with cilia, and in three cases atypical papillary formations with the same morphology as the primary tumor in the cysts. In two cases we found cylindrical cells, single and in clusters, with severe nuclear atypia, that we classified as malignant. All five washings, three with severe nuclear atypia and two with malignant cells, immunocytochemical reaction on epithelial antigen showed positive reaction. All positive peritoneal washings were noticed in serous subtype tumors. Conclusion: In ovarian tumors with surface papillary formations, atypical cells in peritoneal washing is expected. Peritoneal washing cytology is a useful indicator of peritoneal dissemination in cases with ovarian tumors with smooth intact surface, and the way of subclinical peritoneal spread of this tumor cells is open for debate.
