Sažetak
To explore the involvement of proinflammatory cytokines, TNF-α and IL-6, in oral lichen ruber (OLR) local immunoreaction through the cytokine levels in whole unstimulated saliva (WUS). To examine the correlation of cytokine levels and symptoms as well as clinical levels of the inflammation and hyperkeratosis. WUS from 25 patients with the erosive OLR (LRE), 38 with reticular OLR without inflammation (LRP) and 37 age- and sex-matched controls was investigated for the presence of IL-6 and TNF-α by ELISA. Prior to WUS collection the visual analog scale for symptoms and clinical levels of the inflammation and hyperkeratosis were recorded. Salivary IL-6 and TNF-α levels were significantly increased in the LRE patients when compared to the controls. The LRE patients had significantly higher levels of IL-6 compared to the LRP patients. The levels of TNF-α were significantly higher in the LRP patients compared to the healthy controls, but the levels of IL-6 were not. In the LRE patients levels of IL-6 positively correlated with clinical level of the inflammation. We did not find any correlation of cytokine levels with clinical symptoms or levels of the hyperkeratosis. The results of this study suggest different local immunoreactions in different types of OLR and could mean that these cytokines are not involved equally in pathogenesis of the LRE and the LRP. Analysis of proinflamatory cytokines IL-6 and TNF-α in WUS may be useful method for monitoring the local immunoreaction in OLP patients.
