Sažetak
In this prospective study, the clinical course, treatment and behavior of visual function in indirect optic nerve lesion and mild head trauma were observed in 39 patients. Longitudinal follow-up, performed for one year after the injurying, included visual acuity and vision field testing. It is pointed out that serious vision function impairment may also occur in mild head trauma, usually ipsilaterally with the injury. The loss of vision may range from slight blurring to finger counting, light perception or even amaurosis. In such patients, eye biomicroscopy as a rule yields normal finding with transparency of all optic media. Ophthalmoscopic finding is also normal. The testing of photomotor pupillary reactions shows them to be less abundant, tardy or even absent to direct light, while the consensual reaction is normal. Therapeutic approach in clinical practice varies. In this study, medicamentous treatment with methylprednisolone megadoses was chosen. The use of this therapy early after the injury resulted ina a considerable vision function recovery, whereas its use 30-45 days after the injury failed to produce satisfactory results.
