Sažetak
The aim of this study was to determine the prevalence of ESBL-producing Escherichia coli strains isolated from urine of outpatients in Zagreb region, to characterise them and to compare them with ESBL-nonproducing E. coli regarding serum sensitivity, antigen specificity, hemolysin production and adhesins expression. The antimicrobial susceptibility was determined using a standard disk diffusion NSSLS procedure and all isolates were tested for ESBL production by a double disk diffusion technique. The susceptibility to serum killing was measured according to Schiller and Hatch method. All analysed isolates were serotyped using 17 different O-antisera. The production of hemolysin was observed after stabbing bacteria in 5% human blood agar. The expression of adhesins was defined by hemagglutination and inhibition of hemagglutination in microtiter plates. During January to May 2004 a total of 2 451 E. coli strains were isolated. Among them 39 strains produced ESBL (1.59%). Those isolates and 45 randomly chosen ESBL-nonproducing E. coli isolates were further analysed. Serogroup O4, hemolysin production, expression of P- and type 1 fimbriae as well as resistance to gentamicin were all significantly more prevalent among ESBL producing isolates, while serum resistance was more prevalent in ESBL non-producing E. coli. Further phenotypic and genotypic characterisations of these strains are necessary.
