Sažetak
Non-albicans Candida species account for 35-65% of all candidemias and are especially frequent in patients with haematological malignancies. The optimal therapy of candidemia in neutropenic patients is not fully determined. Three patients with haematological malignancies who developed fungemia caused by non-albicans Candida species during period of neutropenia are described. The causing pathogens were C. famata, C. kefyr, and C. tropicalis, respectively. Patients were treated with liposomal amphotericin B (AmBisome) at a dose of 2 to 3 mg/kg daily after they have developed intolerance to conventional amphotericin B deoxycholate. They tolerate therapy with AmBisome very well and pre-existing renal impairment improved during therapy. Therapy was successful in all three cases, clinical and radiological signs of fungal infection resolved completely without recurrence after recovery of neutrophil count. AmBisome seems an efficient and safe therapeutic choice for fungemia caused by rare non-albicans Candida strains.
