Sažetak
Since early 70-ies we established our modification of thyroid surgery and performed more than 6.000 procedures without any considerable changes. The main characteristics of this approach to the thyroid are: incision about the middle of the gland's vertical diameter reaching only medial borders of the sternomastoid muscle. Transection of one or both sternohyoid muscles and proceeding directly to the thyroid without raising upper and/or lower flaps. The next steps are: preparing pyramidal lobe, if exists, dividing small cricothyroid vessels, followed by the upper pole artery and vein and Kocher's vein, then the lobe of the thyroid is turned over the trachea. Recurrent laryngeal nerve is identified and traced to its entry in the larynx. Inferior thyroid veins are cutted. Finally, ligament of Berry is cutted and the lobe and isthmus are separated from the opposite side.
