INFLAMMATORY MARKERS IN ACUTE CORONARY SYNDROME AUTHORS:M.Bergovec,Z.Trstenjak,V.Raos,J.`iki-Vagi,S.Pavlovi, M.Ragu~,}.Romi Department of Cardiology,Dubrava University Hospital,Zagreb,Croatia INTRODUCTION:Coronary artery disease is one of the most frequent causes of death and disabling symptoms in the industrialized world.Therefore it is extremely important to increase our understanding of its development,and all the pathophysyological elements contributing its occurence-this knowledge may eventually lead to significant reduction of the incidence of coronary heart disease in the future.Many studies in the last decade have demonstrated the presence of inflammation at various stages of atherosclerosis,implying that immune system and inflammatory processes also have an important role in coronary disease occurence. AIM OF THE STUDY:The aim of the present study,that our Clinic is conducting.is to analyze and evidence the association of acute coronary syndrome (ACS) and increased levels of certain pro-inflammatory markers in patients serum.Between the variety of acute-phase serum proteins, we decided to determine and follow the dynamics of high sensitive-CRP (hs-CRP),interleukin 6(IL-6),serum amyloid A(S-AA) and neopterin,which are all considered first class acute-phase proteins.The study will analyze,as well,the association of the inflammatory markers and standard,well-known risk factors for coronary heart disease(diabetes mellitus,obesity,hypertension,smoking and serum lipid profile),and also the correlation with serum values of cardioselective enzymes.At the end we intend to prove connection between increased levels of inflammatory markers and severity of heart decompensation. PATIENTS:At least three hundred patients with ACS, diagnosed right after admittance to the coronary unit will be included in the research project.Serum samples are taken at the beggining of hospitalization,24 and 72 hours after the presentation,and then 1,3,6,12 and 24 months after the coronary incident.Samples are stored at 700C until analysed. METHODS:Data on cigarette smoking,diabetes,hypertension and basic laboratory tests are registered for each patient at the presentation. Body mass index (BMI) and waist-hip ratio (WHR) are used as indicators of obesity.Ejection fraction measured by transthoracal ultrasound,pulmonary congestion visualised by chest x-ray and clinical symptoms are taken as main parameters for diagnosis and graduation of heart decompensation.Levels of inflammatory markers in serum samples will be analyzed in duplicate using commertially available ELISA-immunoassay kit.Statistical analysis will be performed using SPSS software program. df   H*OJQJOJQJ CJ OJQJdfHI   0PP&P 1.
