Sažetak
Introduction: Crohn's disease (CD) and colitis ulcerosa (UC) represent chronic inflammatory bowel disease (IBD) of unknown aetiology. Current data suggest that the immune dysbalance is an important factor in the pathogenesis of IBD (1). Dipeptidyl peptidase IV (DPPIV/CD26) is a multifunctional glycoprotein with broad tissue distribution and known activity in serum (2). The role and origin of soluble DPPIV is not completely understood, but it is clear that the enzymatic activity or at least the catalytic domain of DPPIV is indeed involved in the immune regulation, including the T cell activation. Methods: The aim of this study was to determine the serum DPPIV activity in the children with active IBD, compared with the levels in healthy controls. The diagnosis of CD and UC was based on clinical, laboratory, endoscopic and histological data. CD activity was evaluated by using the Paediatric Crohn´s Disease Activity Index [PCDAI]. The study was performed in 25 children with active IBD (CD n=15, [PCDAI]>35 and UC n=10). The control group included 40 healthy children. Determination of DPPIV activity in serum was performed using Gly-Pro-p-nitroanilide as the substrate (3). Results: Serum DPPIV activity in children with active CD (61, 80&plusmn ; ; 14, 37 U/L) was significantly decreased compared with the levels in healthy controls (94, 33&plusmn ; ; 25, 38 U/L) (p<0, 05). It has also been found decreased activity of serum DPPIV in children with UC (67, 12&plusmn ; ; 16, 17 U/L). The levels of DPPIV activity in CD patients were inversely correlated with [PCDAI] score. Conclusions: Our observation that serum DPPIV activity was decreased in IBD patients suggests a functional compartmentalization of DPPIV, which can be interpreted as adaptive systemic immune responses to a local inflammatory reaction. Soluble DPPIV may be involved in the pathophysiology of IBD and appear to be useful as a new disease activity measure for IBD.
