Sažetak
All patients with myocardial infarction or coronary death occuring during the period of five years (1987-1991) were included into the study. The source of the data was population-based acute myocardial infarction register of Zagreb (1 Million inhabitants). Case fatality was calculated by three different definitions: out-of-hospiptal case fatality, 24-hour case fatalityfor hospitalised cases and 28-day case fatality for hospitalised cases. The total number of heart attacks during that time was 7692 and nearly half of them 3758 (49%) died. Nearly two thirds of deaths (63%) occured before the patients reached the hospital and only one third died in the hospital. Among those who reached the hospiptal alaive 24-day case fatality was about 10%. It was higher for women (13%) than for men (8%). 28-day case fatalityfor hospitalised cases was 23%, 30% for hospitalised women and 19% for hospitalised men. Case fatality was higher in women than men, especially for the 28-day case fatality for hospitalised cases. As most deaths occur at home before reaching hospital the only way of reducing it is adequate primary and secondary prevention.
