Sažetak
Hemorrhagic fever with renal syndrome (HFRS) is a systemic infectious disease caused by hantaviruses. With the exception of the coastal region and islands, the whole Croatia is endemic for HFRS, caused by Puumala (PUUV) and Dobrava viruses (DOBV). In March, 2002, in the drug- treatable community « ; ; Susret» ; ; Ivanovac, northeast of Croatia, a man aged 30 years developed severe HFRS with fulminant systemic hemorrhage and acute renal failure, and died. During the March-April, additional 11 cases were confirmed (Miletic-Medved et al Lancet 2002 360, 415-416). More than 350 HFRS patients were serologically confirmed till the end of the outbreak (October) from different parts of Croatia. The majority of the patients had mild to moderate clinical picture and PUUV infection (>95%) and the severity of the diseases was graduated according to the Croatian scale. During the outbreak, rodents sero-survey was done and lungs and kidneys of Clethrionomys glareolus, Apodemus flavicollis and Apodemus sylvaticus were detected for hantavirus antigen. We found that 12% of the tested C. glareolus and 11% of the tested A. flavicollis were positive, while 23% of the tested A. sylvaticus were positive. Additional molecular and phylogenetic analysis are in process for the characterization of hantaviruses which caused outbreak in Croatia in 2002 and their comparison with findings in 1995 HFRS outbreak. Additionally, rapid POC PUUMALATM and POC DOBRAVATM point-of-care test (ERILAB, Finland) immunochromatographic test for the detection of PUUV and DOBV specific IgM antibodies in serum has been evaluated during the outbreak and specificity, sensitivity and diagnostic efficacy has been determined.
