Sažetak
Hemorrhagic fever with renal syndrome (HFRS) and leptospirosis continue to cause significant number of human illnesses worldwide, and both are endemic diseases in Croatia. Clinical pictures of HFRS and leptospirosis can closely mimic each other and patients mostly have fever, headache, myalgia, liver and kidney disorders. Immune mechanisms behind the development of symptoms and lesions induced by hantaviruses and leptospiras are largely unknown. For that purpose we analyzed the role of peripheral blood mononuclear cell (PBMC) phenotype changes, as well as the role of some cytokines in patients with HFRS and leptospirosis. Furthermore, we compared these immunological findings with clinical data in order to determine their diagnostic or prognostic value. Cytokine´s (CCL2 (MCP-1, monocyte chemoattractant protein), CXCL8 (IL-8) and IL-10) production in patients sera was measured by commercial ELISA sets. Whole blood two-and three-color flow cytometry was used for detection of immunophenotypic changes in the main lymphocyte populations and lymphocyte activation markers. Significantly higher levels of CCL2 and CXCL8 were found in patients with HFRS or leptospirosis than in negative controls, but there were no significant differences between the two groups. IL-10 was elevated in about 30% of patients in the early acute phase of both diseases. HFRS patients expressed higher percentage of activation antigens (TCR+HLA-DR+, CD8+CD71+, CD4+CD25+, CD20+CD23+) on lymphocytes than patients with leptospirosis, but the percentage of CD4+ and CD8+ lymphocytes simultaneously expressing both CD45RA and CD45RO markers were higher in patients with leptospirosis. To our knowledge this is the first comparison of immunological parameters in these two clinically similar diseases.
