Sažetak
The aim of the study was to assess plasma levels sICAM-1, BigET-1 and proANP in samples from periferal venous and coronary artery blood in patients with AMI treated by percutaneous coronary intervention (PCI). Twentysix patients (median 58, 5 yrs) admitted to the Coronary Care Unit were studied. Treatment by PCI was started within two hours of pain onset and blood samples were taken from periferal venous (V) and occluded coronary artery (A). Wilcoxon and Pearson tests were used for statistical evaluation. The medians of BigET-1 (fmol/ml) and proANP (fmol/ml) were elevated according to reference values: A=3, 30 ; V=2, 85 and A=2133, 8 ; V=3031, 3 respectively. The medians of sICAM-1 (ng/ml) were in reference interval: A=254, 0 ; V=248, 5. Only proANP values of A samples were statistically different (lower levels) from V samples (P=0, 035). Positive correlation was found for the pairs A-V of all analytes: BigET (r=0, 601 ; P=0, 00116), sICAM (r=0, 641 ; P=0, 004) and proANP (r=0, 799 ; P=0, 00001). The correlation between pairs of A samples for proANP and sICAM was significant (r=0, 505 ; P=0, 00859). Specific local cardiac events showed diminished ANP and sICAM secretion in comparison with periferal levels, probably by higly activated vasoconstriction system induced by strong reduced blood flow distal of occlusion.
