Sažetak
Aim: To analyse possible predictors of orthodontic treatment success (mainly the role of parents) in patients with Class II Division 1 malocclusion treated with removable functional appliances. Materials and Methods: The study included 88 dyads of parents and young adolescents aged 11 to 13 years (47% female) with a Class II Division 1 malocclusion (Class II molar relationship, overjet (OJ) ≥ 6 mm) during CS2, CS3 or CS4 stages of skeletal development. All participants were treated for one year with a randomly chosen removable functional appliance – either Twin block (TB) or Sander bite jumping appliance. Child Perceptions Questionnaire, Parental- Caregivers Perceptions Questionnaire and Family Impact Scale were self-administered. Treatment was considered unsuccessful if patients stopped coming for regular check-ups and if an OJ reduction of less than 40% was detected. Multiple logistic regression was used in the analysis. Results: During the period of one year, a higher proportion of successful than unsuccessful cases was observed (58 vs. 42%). In the unsuccessful group, 46% of subjects voluntarily stopped coming, and 54% continued to come, but no significant OJ reduction was observed. Further analyses have shown that the type of appliance used and the parental perception of functional limitations (FL) of their own child (FL≤4) are the only significant predictors of orthodontic treatment success in cases of Class II/1 malocclusion treated with a functional orthodontic appliance. The amount of OJ, the patient's age, the child's self-assessed oral health and satisfaction with smile aesthetics, and the parents' perception of the aforementioned factors, as well as the impact of malocclusion on the family, did not stand out as significant determinants of treatment success. With the control of gender and impairment of children's social well-being (dichotomized at 1), the chances of treatment success were equally increased by low values of the child's functional limitations noticed by parents (95% CI, 1.2-9.0 ; p=0.020) and TB treatment (95% CI, 1.2- 8.6 ; p=0.016) – a total of 3.3 times. Conclusion: Parental perception of their child’s functional limitations and type of appliance (TB) are significant predictors of treatment success of Class II Division 1 malocclusion with removable functional appliances. Oral symptoms and problems of emotional and social nature reported by parents as well as family impacts are of negligible influence.
