Sažetak
Complex interventional procedures as well as unoptimized interventional procedures can result in patient skin doses that exceed the threshold for the appearance of deterministic effects (radiation-induced skin injuries). Given that skin injuries usually appear long after the procedure (after several weeks or longer), they are often not recognized as injuries caused by radiation and such patients are very often not provided with appropriate health care. For this reason, it is important to establish a system that will enable the identification of patients in whom deterministic effects can be expected after the interventional procedure as well as their follow- up. The identification is based on the comparison of the values of the dose indices and other parameters which are used for the assessment of patient exposure with trigger levels (peak skin dose, kerma in air at reference point, kerma-area product (KAP) and other). In this paper, the system of Dubrava University Hospital for the identification and follow-up of patients in whom skin injuries can be expected will be presented. The system is based on the custom-made patient dose management system (DMS), within which a special module was developed for the identification of patients for whom trigger levels are exceeded. In case of Dubrava University Hospital , trigger levels are taken from the Safety in Radiological procedures (SAFRAD) reporting system established and maintained by the International Atomic Energy Agency (IAEA). Existing standard operating procedures for monitoring such patients will be also presented as well as the results of the analysis of data on patients for whom trigger levels were exceeded before the development of the aforementioned module, including first cases of follow-up.
