Aim: The aim of this study was to investigate the relationship between various aspects of anxiety (trait anxiety, anxiety sensitivity, anxious thoughts and GI-specific anxiety) and indices of attention in irritable bowel syndrome (IBS) patients. Methods: A total of 90 patients and 77 healthy subjects participated in the study and completed the set of questionnaire including Trait Anxiety Inventory (STAI-T), Visceral Anxiety Inventory (VSI), Anxiety Sensitivity Index (ASI) and Anxious Thoughts Inventory (ATI), as well as a modified Stroop task with 4 word categories (GI symptom- related words ; emotion-related words, situation- related words and neutral words). Results: A two-way ANOVA (word category x diagnosis) on median reaction time (RT) data showed that neither word category nor diagnosis had significant effects on RT in IBS patients. A generalized linear mixed model (GLMM) analysis on all available RT data showed that IBS patients were faster than healthy control in their reaction times regardless of word category. Furthermore, significant negative correlations between RT for all 4-word categories with anxiety sensitivity and anxious thoughts was found, but only in IBS patients. Conclusion: Results indicated that IBS patients showed the particular response pattern on Stroop task compared to control – shorter RTs on all word categories and its relation to higher anxiety. These findings point to altered attentional processing in IBS patients, indicating that they might be generally more vigilant (attentive). These attentional characteristics of IBS patients could be relevant for symptom perpetuation and might have practical implications for treatment.
