BACKGROUND-AIM. Determination of vitamin D concentration by liquid chromatography-tandem mass spectrometry (LC-MS/MS) has demanding pretreatment of samples. Therefore, the overall laboratory process is time-consuming and the laboratory report is waited longer compared to immunoassays. The solutions for progressive increase in the number of requests for vitamin D determination has been found in the automated sample pretreatment. The aim of this study was to compare the turnaround time (TAT) for vitamin D in serum after the introduction of automation compared to the period when samples were prepared manually. METHODS. Automated sample pretreatment is performed on the preanalytical module Shimadzu CLAM-2030 by the modified protocol for manual preparation using commercial reagent kit (RECIPE ClinMass® LC-MS/MS 25-OH Vitamin D2/D3). Concentration of vitamin D in serum was determined by the LC-MS/MS (UPLC NEXERA X2-LCMS-8050, Shimadzu), accredited according to ISO 15189. Results for the TAT were obtained from the laboratory's information system. We analyzed the percentage of requests reported within 2 days, 3 days and 7 days. Also, we analyzed the time in which more than 99% of requests were reported. RESULTS. For the period of manual sample preparation (from January to August 2022.) requests were reported as follows: within 2 days 22, 98%, 3 days 43, 38%, 7 days 82, 6% and >99% of requests were reported within 13 days. After implementation of automated pretreatment (from September to December 2022.) requests were reported as follows: within 2 days 54, 98%, 3 days 71, 87%, 7 days 97, 54% and >99% of requests were reported within 8 days. CONCLUSIONS. A comparison of the TAT for the determination of vitamin D concentration in serum after the introduction of automated sample pretreatment compared to the period when samples were prepared manually showed that the time from receiving the request to reporting the findings was significantly reduced. This enables faster notification of clinicians and timely introduction of the necessary therapy, contributing to a more favorable treatment outcome.
