Introduction: Papillary urothelial carcinoma is a type of bladder cancer. It usually starts in urothelial cells in the bladder lining. Urothelial cells also line the urethra, ureters, and kidneys. Low-grade papillary urothelial carcinomas have the lowest risk of progression to invasive disease of all the superficial tumor types, with 50–70% recurrence rate after transurethral resection of bladder tumor. Case report: We present the case of a 69-year-old male with recurrent low-grade urothelial carcinoma of the bladder and the prostatic part of the urethra. Transurethral resection of bladder tumor was performed in 2020. One year after TURBT, ultrasonography showed a suspicious right-sided proliferation of the lateral wall measuring two centimeters. Urethrocystoscopy revealed three renal calculi with base fused to the bladder mucosa and two papillary tumors in the prostatic part of the urethra measuring three millimeters. Atypical cells of the transitional epithelium were found in cytological analysis of urine. Afterwards, a resection of papillary tumors was performed and ordered for pathohistological analysis. Biopsy samples revealed a tumor tissue with multiplied, well-differentiated atypical urothelial cells that occasionally covered villous structures. In June and July of 2022 patient received six doses of intravesical Bacillus Calmette-Guerin immunotherapy. A follow-up urethrocystoscopy showed no signs of recurrence. Conclusion: Intravesical BCG immunotherapy is undoubtedly the most successful treatment for tumor of the transitional epithelium because of high recurrence rate. When BCG immunotherapy is applied as a liquid through a catheter, it supports eradication of the cancer cells by improvement of activity of the immune system cells.
