Introduction & Objectives: In this paper we present the metods of surgical tretment of the urinary stones in our department from the year 2002 to 2009. Material & Methods: From the year 2002 to 2009 we have surgicaly treated 675 patients with the urinary stones. Of these only 5% were treated with the open surgery. All other patients were treated endoscopicaly. Of the endoscopic methods we did: cystolithotrypsy, uretrorenoscopy end lithotrypsiy, PCNL, and laparoscopic pyelolithotomy, pyeloplasty, uretrolithotomy. Results: Conversion rate was 2%, hospitalization after endoscopic treatment is in avg. 3 days, after laproscopic surgery 5 days. The need for postoperative analgesia is significantly lover, in average 2 days after endoscopic treatment and 4 days after laparoscopic treatment. From the year 2002 until today the number of patients treted with the open surgery decreases. Indications for the open surgery are the anatomic variations such as horseshoe kidney, and the size of the stone that one open surgical intevention would provide better results then mulitple PCNL or ESWL treatment end contraindicationes for the PCNL end ESWL treatment. Conclusions: According to our data conclusion is that minimally invasive methods today are taking the primary role in the treatment of the urinary stones, so as in other surgical branches. Anyway, in some cases of the urinary stones there is clear indication for the open surgery. Because of increasing number of endoscopic operations educations in open surgery of the younger generations becomes more difficult
