Sažetak
Cutaneous leishmaniasis is the most common form of leishmaniasis caused by flagellate protozoa of the genus Leishmania transmitted by sand fly bites. Old World leishmaniasis is endemic in the Mediterranean Sea and the neighbouring countries. We present a case of a 37-year-old Syrian man, who lives in Varaždin for the last three years, with a recurrence of lesions at the site of apparently healed disease five years after the original infection on the distal left volar forearm. Clinical aspect was a non-specific inflammatory plaque measuring 3, 5x1.5 cm in diameter and a shallow ulceration covered by crust who persisted for at least several months. According to medical history this lesion was mistreated for other skin diseases, especially bacterial and fungal infections, prior to the referral. Serologic detection of antibodies and polymerase chain reaction (PCR) from the lesion were positive for Leishmania parasites. Histopathological examination of skin bioptate showed granulomatous inflammatory reaction, but amastigota life cycle forms were not found. The patient was cured by fluconazole 200 mg received orally once daily for six weeks. This treatment's choice was based on a few facts: recurrence of lesions with regional adenopathy, size and localization of lesions on the distal left volar forearm for which local treatment is not feasible, and difficult access to pentavalent antimonials which are not registered in Croatia.
