Sažetak
A series of neural disorders can be observed after stroke, and most common is the decrease of cognitive functions. Such changes can be observed in 30–50% patients, seriously affecting their quality of life as well as decreasing their ability to work. After using several neuropsychologicals tests, the conclusion was reached that common effects are deterioration of short term memory, language expression and comprehension, attention. The goal of this research was to determine the frequency of deterioration of short term memory and attention in patients after their first stroke, correlated with known risk factors. In this study we observed 150 first-stroke patients, ages from 50 to 82 years (70 males and 80 females). Methods: Using neuropsychological tests, visuospatial tests and digit span, finger-tapping, MMSE and NIHSS scale, compared to morphology changes shown by MRI, changes were recorded and followed together with changes of their cognitive functions, both one month and six months after stroke. Patients with mild motoric deficit, without vision impairment or a changed mental state were selected in order to enable easier cooperation and more accurate recording of changes of cognitive functions. Results show that one month after stroke about 28% of the patients had changes in their cognitive functions. MMSE ranged 23–28, suggesting mild cognitive deficit, especially in memory and attention, but also in time of reaction. MRI showed defects of white brain substance as well as hemisferal lesions both in temporal or parietal regions, which correlated to severity of deficit. Six month after the first stroke we recorded improvement in cognitive functions in 2% of our patients, while others showed substantial reduction in short term memory and attention, without progression. It is evident that changes of mentioned cognitive functions were also influenced by background, age and other risk factors of cerebrovascular disease. We continue to monitor changes of cognitive functions in our patients. Conclusion: Poststroke dementia is very frequent. Early identifications of people at risk of poststroke dementia is very important to enable intervention and secondary prevention.
