Sažetak
Epileptic seizures caused by abnormal cerebral electrical discharge may be symptomatic or asymptomatic. A careful clinical history taken from both the patient as well as any witnesses to the clinical events (semiology) is the most critical aspect of making an accurate diagnosis. Seizure semiology represents a complex interaction between brain, body, and environment, which is caused by more or less direct transient alteration of cerebral network dynamics and is fundamental to elucidating how they are anatomically generated in the brain. The basis for semiological analysis is essentially pattern recognition by expert clinicians. Level of expertise is crucial, because this affects the likelihood of rare or subtle clinical patterns being recognized: accuracy of semiological diagnosis depends both on level of clinical expertise and complexity of clinical pattern. Considering epilepsy as a network disorder seizures and their semiological expression represent complex dynamical systems that can be described in spatial and temporal terms. A better understanding of structural and dynamic network properties may improve epilepsy diagnosis and treatment. In defining the epileptogenic network in addition to semiology, we are also assisted by video-EEG, brain. MR, interictal PET CT brain, ictal SPECT and neuropsychological evaluation. The recently findings indicate that the epileptogenic network is also influenced by subcortical structures, basal ganglia and thalamus. Appropriate definition of the epileptogenic network. taking into account semiological data, is necessary in planning further pre-surgical processing in treat- ment patients with drug resistant epilepsy.
