Sažetak
Introduction: Patients with pharmacoresistant epilepsy are certainly potential candidates for neurosurgery. However, significant number of patients from this group has so called "MRI negative epilepsy or epileptic lesions near cloquent cortex and therefore they are sometimes not good candidates for neurosurgery. However, with adequate preoperative evaluation they might be considered as potential neurosurgery patients. Case report: We will present two patients with negative brain MRI as well as negative post- processing of MRI findings but with a clear clinical picture of temporal lobe epilepsy. They have been successfully operated following preoperative evaluation that included brain SPECT, PET-CT and semi-invasive video EEG monitoring using sphenoid electrodes. The third patient with a large developmental malformation of the brain, near eloquent cortex has also been successfully operated after adequate preoperative procedure. PHD analysis of first two cases showed hippocampal sclerosis type I and III and in the third patient focal cortical dysplasia type II b). Conclusion: The above presented cases show the high importance of proper preoperative evaluation of patients with refractory epilepsy, even with MRI negative type as well as patients with large lesions located near eloquent cortex
