Sažetak
A 46-year-old male who received a living-donor renal allograft 14 years ago and has an immunosuppressive protocol consisting of tacrolimus, mycophenolate mofetil and prednisone 10 mg for the treatment of chronic active antibody-mediated rejection, presented with high- grade fevers (up to 40°C) 2 weeks after he received the second dose of mRNA vaccine (Pfizer- BioNTech). Initial evaluation revealed no radiologic signs of pneumonia and he was treated as an outpatient. However, 10 days later, his temperature increased to 39°C and he was subsequently diagnosed with bilateral COVID- pneumonia requiring hospital admission. Chest X- ray on admission showed extensive, bilateral consolidation suggestive of multifocal pneumonia. Medical therapy was initiated with remdesivir and ceftriaxone for concern for possible superimposed bacterial pneumonia. Mycophenolate mofetil was discontinued for 7 days with increase the dosage of steroid. Patient was discharged from the hospital in good condition with stable renal allograft function.
