Sažetak
Griseofulvin is the standad treatment for tinea capitis, however the required long treatment (6-8 weeks) may hinder compliance. The possibility that shorter courses of terbinafine are effective was investigated in 2 multicenter, Phase II, randomized, controlled durationfinding trials in 268 children with tinea capitis. At doses extrapolated from the adult literature (3-6 mg/kg), terbinafine was effective, safe and well tolerated. Complete cure rates were 57 for Trichopyton tinea capitis ( 4 weeks treatment) and 62% for Microsporum (6wks). The effect of treatment duration on cure rates was modest. We developed a logistic model, based on data from the 2 trials, to investigate a potential dose - response relationship. Pharmacokinetic data indicate that children have a high clearance rate, thus doses to 10 mg/kg would result in a level of exopsure which is comparable to adults treated with 250 - 400 mg/day. Dose-response modelling showed a significantlinear relationship (p= 0, 03) between complete cure rates and terbinafine dose. Patients treated with doses > 4, 5 and < 4, 5 mg/kg achived cure rates of 53, 9% and 43, 5% respectively, regardless of treatment duration (T and M. species combined). The model predicted complete cure rates of 77-87%at doses 9-12mg/kg. This stady sugegests that complete cure rates of > 75% may be achieved by increasing terbinafine dose while remaining within an acceptable safety profile and possibly, with shorter treatment durations thangriseofulvin. Further studies are required to confirm these findings.
