Sažetak
Chronic hepatitis C (CHC) is an important public health problem and a significant factor for the development of liver cirrhosis and hepatocellular carcinoma. A significant part chronically infected patients enter care only after development of advanced fibrosis. The aim of this study was to analyse changes in fibrosis stages in patients with CHC who achieved sustained virological response (SVR). The stages of fibrosis (F1-F4) were determined by elastography (FibroScan) before antiviral treatment and after SVR. Demographic, laboratory and clinical characteristics of the patients were analysed. Out of a total 183 patients with CHC, 55.7 % were men (median age 47 years, IQ 18-83). Almost all patients (178/183, 97.2%) achieved SVR 12 and/or 24 weeks after the end of antiviral treatment. Majority of patients with subtype 1a were men (62.3%) (median age 46, IQ 24-76) and all achieved SVR. Prior to treatment initiation, F1 fibrosis stage was determined in 31% of patients, F2 in 12.5%, F3 in 12.5%, while 44% of patients had F4. After treatment, 50% of patients had F1, 6% of patients had F2 and 6% of patients had F3, while 38% of patients had F4 stage of fibrosis. Furthermore, majority of patients with subtype 1b were females (57.3%) (median age 57 years, IQ 18-83). Prior to treatment initiation, F1 fibrosis stage was determined in 20% of patients, F2 in 28.8% of patients, F3 in 8.7% of patients, while 42.5% of patients had F4. After treatment, 58% of patients had F1, 21% of patients had F2 and 2.6% of patients had F3, while 18.4% of patients had F4 stage of fibrosis. Finally, majority of patients with subtype 3a were men (75.6%) (median age 48 years, IQ 33-69). Prior to treatment initiation, F1 fibrosis stage was determined in 16.7% of patients, F2 in 25% of patients, F3 in 8.3% of patients, while 50% of patients had F4. After treatment, 37.5% of patients had F1 stage of fibrosis, no patients had F2, 25% of patients had F3, while 37.5% of patients had F4. The use of antiviral drugs and the achievement of SVR in patients with chronic hepatitis C is associated with a decrease in the fibrosis stages in majority CHC patients.
