Sažetak
Altered expression of various biological response modifiers plays an important role in the immunopathogenesis of chronic hepatitis C (CHC). The aim of this study was to analyse the influence of treatment-induced eradication of hepatitis C virus (HCV) infection on the serum concentrations of cytokines and growth factors associated with inflammation and/or patogenesis of fibrosis. Serum concentrations of 12 cytokines (IL-5, IL-13, IL-2, IL-6, IL-9, IL-10, IFN-γ, TNF-α, IL-17A, IL-17F, IL-4 i IL-22) and 13 growth factors (Angiopoietin-2, EGF, EPO, FGF-basic, G-CSF, GM-CSF, HGF, M-CSF, PDGF-AA, PDGF-BB, SCF, TGF- α, VEGF) were analysed in 56 CHC at baseline, after 4 and 8 weeks of treatment with direct acting antivirals as well as after sustained virological response (SVR) by using bead-based flow cytometry. Statistical analysis was performed using R. Paired comparisons were performed with Friedman's test in case of multiple groups and Wilcoxon signed-rank test in case of two groups. Variable correlation was analyzed with Spearman's correlation coefficient. Concentrations of IL-10 in CHC patients were significantly different at four measurement points (p=0.0032). Significantly higher IL-10 concentrations were observed before treatment (median 1.12pg/mL) compared with concentrations at 4 weeks (median 1.12pg/mL), 8 weeks (median 1.05pg/mL) as well as at SVR (median 1.05pg/mL). Serum concentrations of several growth factors exhibited a characteristic pattern with significant differences at four measurement points observed for epidermal growth factor (EGF, p<0.0001), erythropoietin (EPO, p=0.0215), hepatocyte growth factor (HGF, p=0.0006), stem cell factor (SCF, p=0.0368) and vascular endothelial growth factor (VEGF, p=0.0090). Fibrosis regresion after SVR was observed in 41% of patients. Treatment-induced eradication of HCV is associated with decreased expression of immunoregulatory cytokine IL-10 but the biological background of this observation must be further investigated. Increased levels of several growth factors, in particular HGF and EGF observed during antiviral treatment, might be associated with repairment of hepatic injury by remodeling extracellular matrix and recruitment of cells to the site of injury leading to fibrosis regression in a proportion of patients.
