Sažetak
Application of humeral nail in treatment humeral shaft fractures is a known method of fracture fixation. It used ever more in the nineties of the last century. Its application in the University Department of Traumatology of the Sestre milosrdnice University Hospital Center began in 2001. The operative procedure performed by using a minimally invasive technique without fracture opening to keep up good conditions for fracture healing including hematoma management. However, in the everyday practice it is often not possible to get satisfactory fracture reduction using the closed procedure, so that open reduction and added fixation with wires or screws are mandatory. Over the last 13 years, fixation of fractures with the locking nail made in 406 patients. Cases of pseudarthrosis and pathological fractures were also managed successfully. There were more female than male patients. The mean time from injury to surgery was 2.4 days. Surgery performed immediately after admission to the emergency service when possible. This study comprised 406 patients with humeral shaft fractures treated with the humeral nail (anterograde insertion applied in 231 and retrogarde in 175 patients). The aim of the study was to stress complexity of proper operative treatment of humeral shaft fractures using intramedullary fixation as well as importance good reduction and stable fixation.Although the evidence quality was poor, the results suggest that compared with locking plates, intramedullary nails may be a better choice for the repair of humeral shaft fractures.
