Introduction: Calprotectin and T helper (Th)1/Th17 inflammation of the intestine play an important role in pathogenesis of spondyloarthritis. Local antigen presentation in the context of interleukin (IL)-12 stimulates the production of proinflammatory Th1 lymphocytes, while the surrounding with IL-23 induces Th17 lymphocytes. Activated lymphocytes populate the joints, cause inflammation and joint tissue remodeling, which involves matrix metalloproteinase-3 (MMP-3) activity. The oral cavity is part of the digestive tract, but there is scarce data on inflammatory substances from saliva that could reflect temporomandibular disorder (TMD). The aim was to investigate the proportion of patients with spondyloarthritis who have disc displacement (DD), to determine fecal calprotectin, as a laboratory parameter of intestinal inflammation, salivary IL- 12/IL-23 p40 and MMP-3, and then to correlate these parameters with each other and with restriction of the mandibular movements. Patients and methods: Patients with spondyloarthritis were examined according to the Diagnostic Criteria for TMD form and the restriction of mandibular movements was determined by the Jaw Functional Limitation Scale-20 (JSLF- 20). We used ELISA for quantification of fecal calprotectin, and salivary MMP-3 and IL-12/IL-23 p40. Results: We diagnosed DD in 50.7% of patients with spondyloarthritis and 30.7% of patients were without TMD and served as a control. In patients with DD, the concentration of fecal calprotectin was higher than in patients without TMD, while the concentrations of MMP-3 and IL-12/IL-23 p40 did not differ. Only in patients with DD did calprotectin positively correlate with the IL- 12/IL23 p 40. Additionally, the IL-12/IL23 p 40 positively correlated with MMP-3. Patients with DD had greater limitation of mandibular movements and MMP-3 positively correlated with JSLF-20 scale scores. Conclusion: Patients with spondyloarthritis and DD have higher concentrations of fecal calprotectin, which is positively correlated with the proinflammatory marker of Th1/Th17 immune response in saliva. Immune response supports restriction of mandibular movements mediated by MMP-3.
