SummaryNeoadjuvant chemoradiotherapy is a well-established standard treatment for locally advancedrectal cancer and has led to a remarkable improve-ment in local control. However, distant recurrencesstill pose a notable threat and local failure, albeit in-creasingly rare, can lead to unfavorable clinical situ-ations. In this short review, we discuss three promis-ing new strategies to improve rectal cancer treatment:total neoadjuvant therapy, short course radiotherapy, and immune checkpoint inhibitors.
