Sažetak
Aim: The study aimed to compare the differences in intensity of burning pain sensation and salivary opiorphin levels, endogenous opioid with strong analgesic properties, following stimulation with capsaicin in patients with previously diagnosed temporomandibular disorders (TMD) and healthy age- matched subjects. Materials and methods: The study involved a total of 25 subjects (11 TMD patients and 14 healthy volunteers). In both groups the intensity of burning pain sensation elicited by a series of capsaicin-soaked disks was recorded by Numerical pain rating scale (NPRS). Saliva sampling opiorphin levels was performed on three time points, before and twice following the stimulation. Opiorphin levels were quantified by HPLC-MS/MS. Results: Burning pain sensations changed significantly during the stimulation time but without significant differences between groups (chronic TMP patients p<0.0001 ; control group p<0.001). There was an almost identical pattern of response to the burning painful stimulus in both groups, with parallel pain intensity curves over the observed period. A statistically higher levels of opiorphin at baseline were observed in chronic TMD patients, compared to control group (p<0.05) and the same relation was observed in the second and third sampling, but without statistically significant differences between groups (p>0.05). In patients with chronic TMD, an increase in opiorphin levels was observed following the painful stimulation, but not sufficient to be considered statistically significant (3rd – 1st p=0.68, 3rd – 2nd p=0.40), while in the control group the levels of opiorphin remained almost unchanged during all samplings. Conclusions: Although the subjective response to the experimental stimulus with capsaicin did not differ between study groups, the response of the organism, in terms of secretion of higher concentrations of opiorphin in chronic TMD patients, may reflect an involvement in the development and maintenance of temporomandibular pain.
