Sažetak
Difficult airway is a millstone in every day practice in anesthesiologist work. Guidelines in difficult airway are raising awareness for additional ways of managing airway, but in many cases of ent and maxillofacial surgery with shared airway they are not always easy to follow. In this work we want to represent 4 cases of difficult airway and the way we managed them. Case report: Case reports: First is a patient with inoperable carcinoma of unknown origin, who came to our department from oncology department because of dyspnea, syndrome vena cava superior, respiratory insufficiency. Emergent tracheostomy was indicated so he can continue with his chemotherapy. Second, is a case of a newborn baby with a major congenital heart malformation, severe pulmonary hypertension and anamnesis of difficult intubation presented for us for tracheotomy because of progressive respiratory insufficiency. Third case is a patient with idiopathic laryngeal stenosis and reduced laryngeal inlet to 5 mm only. And last is a patient with massive retrosternal goiter and compressive symptoms presented for thyroidectomy. Discussion: Managing shared airway requires close collaboration of skilled anesthesiologist trained in advanced airway management and surgeon for safe airway and adequate surgical access.
