Sažetak
The aim of the paper is to show the impact of clinical pharmacy on rational pharmacotherapy, including pharmacoeconomic aspects, patient safety and the overall quality of health care in hospitals, and to establish the role of clinical pharmacists in Croatia. Significantly better therapeutic outcomes, with measured parameters, were recorded as a result of pharmacists' activities. Participation of clinical pharmacists in the medical team shortens the length of hospitalization, reduces the need to return to intensive care units or readmissions and lowers total and drug costs. Favorable safety outcomes were recorded in the reduction of adverse drug reactions and medication errors. Favorable outcomes such as patient compliance, knowledge and quality of life were also found. Results of the cited scientific papers indicate that active participation of clinical pharmacists in the medication cycle, from admission to discharge, can provide adequate pharmaceutical care, ensure the accuracy and appropriateness of therapy through medication reconciliation, whilst contributing to better health outcomes and reduced mortality, greater safety and patient compliance and reduction of health care costs.
