Sažetak
Background: Genetic susceptibility factors are known to be important in the development of ankylosing spondylitis (AS). Since HLA-B27 can explain only 16 to 50% of the total genetic risk for the disease, others, as interleukin-10 (IL-10) has been emerged as key candidate in predicting the risk and severity of the disease. IL10 being important immunoregulatory cytokine is polarized along diverse axes of response such as Th1, Th2 or Th17. Secretion of IL10 is 75% genetically determined. The highly polymorphic IL-10 gene in the promoter region contains two CA-repeat microsatellites (IL10.G and IL10.R). The heritable differences in IL- 10 production have been shown to be relevant for disease susceptibility and severity. Objectives: To investigate potential associations of IL-10 microsatellite G promoter polymorphisms with susceptibility to AS. Methods: From genomic DNA, IL-10 microsatellites G (IL10.G) were typed by polymerase chain reaction, automated fragment length analysis, and restriction fragment digestion in 53 Caucasian German patients with AS, all B27-positive and 40 HLA–B27– positive healthy Caucasian German controls. AS patients genotypes of IL10.G were correlated with clinical features of the disease such as uveitis or peripheral arthritis. Results: IL10.G9 allele was significantly more frequent among the AS patients than among the controls (allele frequency 0.642, P=0.005, odds ratio 0.301, 95% CI 0.128-0.710). There was a significant decrease in the promoter alleles IL.G8 (allele frequency 0.132 ; P =0.006, odds ratio 3, 943, 95% CI 1.421-10.943) and IL10.G7 (allele frequency 0.038 ; P =0.017, odds ratio 6.375, 95% CI 1.273-31.934) in the AS group compared with the HLA–B27– positive controls. Uveitis developed significantly more frequently in AS carries of the IL10.G9 allele (allele frequency 0.150, p=0, 032 ; OR= 3, 611, 95% CI 1.088-11.084) and arthritis in IL10.G13 allele (allele frequency 0265, p=0, 011). Conclusion: IL10.G polymorphisms are associated with AS and might contribute to increased or decreased susceptibility to AS. Alleles IL10.G8 and G7 microsatellites alleles show a protective effect against the development of AS in the German subjects investigated here. Allele IL10.G9 seem to be a risk factor for the development of AS. This protective effect of variant promoter alleles could be related to differences in IL-10 production and may be clinically relevant.
