Sažetak
Aim: In children with skeletal dysplasia angular deformities of lower extremities are frequent. So far, results of corrective osteotomies were unpredictable due to many and different reasons. We selected group of children with skeletal dysplasia in whom growth plate was not primary affected by dysplasia. The purpose of this preliminary study is to evaluate our initial experience with guided growth technique with aim to avoid corrective osteotomy in children with rare diseases. Methods: Medical records and X-rays of patients who were treated for knee and/or ankle joint deformities between October 2006 and December 2007 were analysed. Preoperative and postoperative angulations at the site of deformity were measured and difference was noted. Operative time as well as complication of surgery was registered. Results: There were nine patients (six boys) with 15 limbs (9 tibias ; four at ankle) treated with “8-plate” technique (Orthofix, Verona, Italy). The average age of the patients was 9 y. 9 m. (range: 5 y 6 m – 15 y 0 m) with the average follow up 12 months (range: 4-23). The average correction achieved was six degrees (range: 0- 11). The operative time was shorter as result of learning curve, with range: 60 minutes (in the beginning) and 17 minutes as experience was better (average: 37 min.). There was one patient who needed re-operation due to the soft tissue i.e. ilio-tibial band compression by “8- plate”. Full correction was achieved in one patient with bilateral knee valgus and implants were removed. Conclusion: Our experience with “8-plate” technique has shown encouraging results in patients with spectrum of skeletal dysplasias. All patients, but one, in our series has shown gradual correction. Learning curve was short even in this group of patients with rare diseases.
