There have been a number of recent publications on the efficacy of botulinum toxin type (BTX-A) as a prophylactic tratment for chronic tension- type headache (TTH) and migraine. Short-term studies demonstrating the ability of BTX-A to reduce the pain associated with headache have already been reported. The aim of the study was to evaluate the long-term effect of BTX-A as a prophylactic treatment for chronic TTH. Thirty patients with chronic TTH resistant to standard therapy were enrolled in this 28-month prospective study. Firstly, tenderness of pericraniale muscles was evaluated by the manual palpation methods (frontalis, temporalis, trapezius, sternocleidomastoideus/mastoid process) for total tenderness score (TTS). BTX-A as BOTOX (dose range 40-95 U) was injected into the most tender pericranial muscles every 3 months during an 18-month period. Clinical assessment (TTS) and home diary kept by eacha patient were evaluated at 4-week intervals during 18 month. In total, 28 patients (17 female, 11 male) completed this 18-month study. These patients experienced a statistiacally significant decrease in TTS (p<0.001) over the 18-month period. There was a constant and cumulative trend of improvment in the number of headache- free days (p>0.001). No major side effects were reported during the study. Study results support the conclusion that BTX-A may be an effective prophylactic therapy for chronic TTH. However, additional studies are warranted to determine the most successful treatment paradigms.
