Sažetak
Population differences in the metabolism of analgesic drugs are a significant factor to be considered when analgesic therapy was prescribed. Critically ill patients have several characteristics that may influence a performance of analgesic therapy. The most important is severity of disease that is the cause of the admission to the intensive care unit (ICU). Tramadol is commonly used opioid analgesic in ICU. It is metabolized by enzymes CYP2D6 and CYP3A4 to more potent and active opioid metabolites, particularly the O- demethylation product M1. Differences in it’s metabolism may result in excessive or insufficient effect, drowsiness, nausea, vomiting and hypotension. In this presentation, differences in in the drug effects and metabolism between tramadol, morphine and other opioid drugs used for acute pain relief in the ICU will be analyzed. A correlation of patients’ comorbidities, with side effects of opioid drugs in the ICU based on our own measurements and data analysis from available literature will be also discussed.
